The COMLEX-USA Level 1 exam is taken by osteopathic medical students in the United States. Specifically, it is taken by:
- Second-year students at osteopathic medical schools across the United States. Most students take COMLEX Level 1 at the end of their second year of med school.

The COMLEX-USA consists of three levels:
- Level 1
- Level 2 Comprehensive Osteopathic Medical Self-Assessment (COMSAE)
- Level 2 Performance Evaluation (PE) - (This is a hands on assessment - not a multiple-choice “exam”)
- Level 3
Level 1 is typically taken during the second year of medical school, Level 2 is taken during the clinical years, and Level 3 is taken during residency training.
The COMLEX-USA Level 1 exam assesses an osteopathic medical student's basic science knowledge, principles of foundational osteopathic medical practice, and ability to apply knowledge to solve clinical problems.
